carbamazepine
Initial dose 200 mg/day, divided bid; increase by 200 mg/day to $0.08/day
Maintenance 600-800 mg/day, divided bid to qid $0.24-0.32/day
or baclofen 5 mg, tid; increase by 5 mg tid weekly up to 60 mg/day divided tid $0.44-1.74/day
Second-line therapies:
phenytoin 300 mg, hs $0.19/day
or pimozide 2-12 mg, once daily $0.32-1.77/day
Additional instructions and notes
-Carbamazepine and baclofen act synergistically when combined, but side effects (drowsiness, nausea or vomiting) may be made worse.
-Patients who fail to respond to medical treatment should be considered for surgery.
